  • Analyzing the Data: Detecting patterns and trends to formulate hypotheses about the root cause. Among its fundamental tools are “The Five Whys”, a simple yet profound technique of asking ‘why’ repeatedly to delve deeper into the problem. he Ishikawa Diagram, or Fishbone Diagram, is another valuable tool, aiding in the categorization and identification of potential causes.
  • Formulating and Testing Hypotheses: Investigating further to confirm or disprove these hypotheses.
  • Identifying the Root Cause: Pinpointing the primary underlying cause.
  • Implementing Corrective Actions: Taking steps to address and rectify the root cause.
  • Verifying Effectiveness: Monitoring the situation to ensure the effectiveness of the implemented solutions.
